“Living Well, Planning Well” Toolkit

The Canadian Hospice Palliative Care Association is sharing their “Living Well, Planning Well” legal toolkit resource with CBA members.

The "Living Well, Planning Well" toolkit for lawyers was updated in fall 2021 to reflect changes in legal terminology and provincial legislation.

The toolkit was developed by the Speak Up initiative in partnership with legal experts, and funded by Health Canada, as part of a three-year project to help people living in Canada prepare for their future health care needs.

This toolkit helps lawyers and their clients through the process of Advance Care Planning (ACP) and its important role within future life planning, and includes other material that lawyers can use to start important ACP conversations with their clients.​

Most importantly, the toolkit can help lawyers make a significant difference in their clients’ lives by encouraging them to reflect on and discuss their clients' wishes and what is most important to them.
